2. Start Page:

3. Add a New Client (patient):
A new client needs to be added to the system. We need only the most basic information to create the client file. |

4. Select Existing Client File:
Clients already in the system are selected from a list. The list can be filtered by first initial. |

5. Client Data Main Page:
The key data for a selected client is displayed here along with options for adding and changing the remaining client data. |

6. Client Data - Step-By-Step:
The client's personal, medical and financial data can be entered in a step-by-step "wizard." Most steps, covering items that are rarely relevant to a particular client are not displayed unless requested. For most clients only three or four steps are required. |

7. Client Data - Expert's Form:
For fast data entry the "Expert's Form" formats the same data fields as above into a single form optimized for tabbing between fields. Experienced users often prefer to move quickly through the familiar data fields.
The Expert's Form and the Step-By-Step both send data to the same place in the database. A user can use either screen or both, if desired. |

8. Script Page - Setting the Script Date:
To define a script one need only select the client, doctor, drug and date from easy lists. The image shows the date selector. |

9. Script Page - Ready to Get the Form:
Once the script is saved you can fillout the drug program application form. The "big-blue-button" gets the form. |

10. Form Cover Sheet:
Each form printed comes with a cover sheet to aid the user. The cover includes key tracking information, a handy checklist, and notes and tips concerning ths particular form. If there have been any recent changes to this manufacturer's form, we;ll let you know. |

11. Client Take-Home Page:
This customizable page is intended to be given to the client to take home. It includes some key numbers and names that will help locate the script record if the client has a question or a problem. It also can supply explanatory material.
While many clients are not good readers, our customers tell us that they often bring materials like this to someone in their community to read for them. Our early customers requested this addition to help keep their patients informed. |

12. Manufacturer's Application Form:
This is the exact form supplied by the manufacturer, filled with the required information for this client. PDA contains hundreds of such forms covering over 2000 medications.
The form (and the coversheets) are sent to your browser as a PDF document, for easy printing and, if you wish, saving to your local drive. |

13. Saved Script Page:
We store information about every script entered into the system, and alow the advocate to track the status of that drug request. Previously saved scripts can be recalled to the "active" page to revise or reprint is needed. You can track your total activity for each client, and for all clients. |
